To complete the online form you need to provide appropriate documentation to enable the Central Credit Register to perform ID verification. This includes:

We will need proof of identity to proceed with a request. Please read the full list of identification documents which we can accept.

On the “my Request” page, a description is available for each type of request that can be made. The options are as follows.

  1. Credit Report: This request type is used to access your Credit Report from the Central Credit Register.
  2. Insert an Explanatory Statement: This request type is used to provide a statement of not more than 200 words relating to any information held on the Central Credit Register which relates to you.
  3. Provide Notice of Suspected Impersonation: This request type is used to provide notice of Suspected Impersonation on your Credit Report, if you reasonably believe that you may have been, are currently being or may be about to be impersonated by any person.
  4. Amendment: This request type is used to request an amendment to credit information contained in your Credit Report.

When the request has been completed correctly a "Confirmation" page opens that summarises the instructions to follow to complete the request. In addition, a window will open in your browser which contains a copy of the application form to be signed and sent to the Central Credit Register with your ID documents. If this page does not open, you can download the application form from the "Confirmation" page by clicking the "Download the PDF form" button. If you chose to receive responses via email you will also receive an e-mail confirmation of submission with the application form attached.
